A Real Consultation Case of a 6th-Grade Girl with Labial Hypertrophy.
• Shape: Wing-type overlapping folds, clitoral hood folds, with mild vaginal folds
This consultation involved an elementary school student who had experienced menarche,
but whose secondary sexual development was not yet fully complete.
The patient was an 11-year-old girl who visited the clinic accompanied by her mother.
She reported significant discomfort, stating that she was unable to run comfortably,
felt constant friction while walking,
and experienced pressure and pain even when sitting.
Because of these ongoing symptoms,
the patient herself expressed a strong desire to undergo labiaplasty,
saying that she wished to proceed with surgery even if revision surgery might be needed in the future.
This was a case in which the decision was voiced clearly and voluntarily by the patient.
Because the labia minora were enlarged and had firm overlapping folds,
including thickened clitoral hood skin,
the incision was designed to allow the tissue to be reshaped smoothly and evenly without excessive contours.
The patient wished for a tight and well-contained appearance within the labia majora,
so the remaining normal labial tissue was conservatively designed
to be less than 1cm in width.
Younger patients often report more postoperative discomfort;
however, this student tolerated the pain well
and experienced a smooth and uneventful recovery.
*Postoperative Day 1:
Pain was mild, and she was able to return home independently after her follow-up visit.
*Postoperative Week 1:
She shared, “I feel much more comfortable now, and sitting is no longer painful.”
*Postoperative Day 20:
She returned with her mother to review the before-and-after photographs.
After the one-week regenerative care session,
she felt so comfortable that she honestly admitted
she had not been doing sitz baths as frequently as instructed—
which earned her a bit of gentle scolding from her mother. ^^
The student cheerfully promised that she would attend her after-school classes regularly
and study diligently, explaining to her mother,
“I couldn’t do the sitz baths because I was busy going to my academy!”
Her response was both endearing and confident.
We had explained in advance that surgery during puberty is uncommon
and that anatomical size may change slightly with continued growth.
The student had already understood this clearly before proceeding.
Although she is a minor,
the decision to proceed with surgery was made with full respect for her own expressed wishes,
and seeing her smile with satisfaction after surgery
was deeply heartwarming for me as well.
Students today are remarkably thoughtful and mature.
This student, in particular, is remembered as someone who was decisive, honest, and self-aware beyond her years.

It can affect daily comfort, hygiene, and self-esteem.
If a student reports similar discomfort,
it is better to seek accurate evaluation and professional consultation
rather than enduring symptoms or waiting without guidance.
Especially for patients in puberty,
a careful and sensitive approach is essential.
Respecting the student’s feelings and personal wishes,
while having open and thorough discussions with the guardian,
helps ensure that the safest and most appropriate decision is made.
