Hello,
Today, I would like to share a patient case from
our clinic and discuss a common question:
“Do I still need a gynecologic examination
if I have never been sexually active?”
This patient had previously visited
our clinic for labiaplasty surgery.
During her preoperative blood tests, signs of anemia were identified,
and we recommended additional gynecologic evaluation.
The patient then asked a question many women hesitate to bring up:
“Doctor, I’ve never had sexual intercourse.
Do I still need a gynecologic examination?”
This is actually one of the most common questions we hear in the clinic.
Many people associate gynecologic examinations
with internal vaginal examinations,
so they assume that if they have never been sexually active,
gynecologic check-ups are unnecessary.
However, gynecology involves much more than
conditions related to sexual activity.
Many common women's health conditions
can occur regardless of sexual experience, including:
• Uterine fibroids
• Ovarian cysts
• Polycystic ovary syndrome (PCOS)
• Painful or irregular menstrual periods
• Heavy menstrual bleeding associated with anemia
• Vaginitis and vulvar itching
• Vulvar skin conditions

In this patient's case, an abdominal ultrasound was performed,
which revealed an ovarian cyst measuring more than 3 cm on the right ovary.
After additional blood tests, we explained that
surgery was not necessary at this time.
However, the cyst would require regular follow-up monitoring.
The patient told us that she had assumed everything was
fine because she had never been sexually active.
After the examination, however,
she better understood the importance of gynecologic screening
and knowing her current health status.
It is important to remember that not every gynecologic examination
involves an internal vaginal examination.
For patients who have never been sexually active,
evaluations can often include:
◇ Abdominal ultrasound
• Blood tests
• Urine tests
• External vulvar examination
These methods allow us to assess many gynecologic
conditions while maintaining patient comfort.
